Below is everything you need to know to prepare for your first visit, and beyond!

New Patient Paperwork

If you would like to save yourself time at your first appointment, please give our office a call at (239) 936-5545 and we will email it to you so you can print and fill out the paperwork. Please be sure to fill out the New Patient Intake as completely as possible. If you have any questions in regards to the paperwork, please let us know at your appointment.

Insurance

We accept most insurance. If you have insurance and would like for us to verify your chiropractic benefits, please click here and fill out the form or call our office at (239) 936-5545. We will be happy to verify your coverage and get right back with you to schedule an appointment at your earliest convenience.

Due to the changes in healthcare, many insurance companies are now requiring additional paperwork. Please be sure to provide the front desk with your specific insurance information and be prepared for a few more forms upon arrival of your first visit.

What To Expect

First Appointment

Your first appointment will include an in-depth health history intake, chiropractic examination, and consultation. We will discuss your goals, the different techniques and protocols we will use, and we will set a realistic timeline for you to see results from the treatment plan. In many cases, we will start your treatment at your first appointment, so you can walk out feeling a bit better than you did an hour earlier!

Ongoing Treatment

Accidents & injuries: You may have several visits per week plus at-home rehabilitative stretches and exercises as your body heals, followed by regular adjustments to ensure long-term healing.

Chronic illnesses: If we are treating chronic illnesses like asthma or migraines, we’ll create a custom, ongoing treatment plan that will include healthy lifestyle recommendations to support your specific condition.

Preventative ‘maintenance’: Many patients practice proactive health with a monthly adjustment to reset and realign their bodies and help them achieve their highest quality of life.

Helpful Resources

We encourage patients to educate themselves about health topics that are important to them. Whether that’s getting better sleep, following an elimination diet, or improving their posture. We’re always sharing resources, tips, and news to help you live your best life.
